Why Refer to Dr. Jetty

Board-certified Internal Medicine physician performing every exam personally — no technician-led intake, no mid-level handoff.
USCIS-designated Civil Surgeon, current with the latest Technical Instructions (2024 adult IPV rule, updated STI/TB age-based screening, MenACWY requirements).
Same-day sealing whenever possible. Dr. Jetty signs and seals Form I-693 the moment all required documentation is in hand, so your client is not waiting weeks for a form to come back.
Intake forms sent in advance, completed before the visit — accelerates the appointment and reduces errors.
Advance CXR prescription provided at the time of the exam, so if TB screening is positive the client goes directly to imaging without a return trip.
Transparent flat fee: $550 per applicant, no surprise billing. Outside labs, vaccinations, and imaging are the client’s responsibility and billed directly by the lab/pharmacy/imaging center.
Clear, respectful communication throughout. Your client leaves feeling cared for, not processed.

The Referral Process

You refer the client. Call (609) 874-0070 or email info@pmacmd.com with the client’s name and preferred contact method. We will reach out to them directly.
We send intake forms. The client receives the vaccination intake and medical history forms by email, completes them at home, and returns them before the appointment.
The exam. Dr. Jetty personally conducts the full examination, orders any required labs or vaccines, and provides prescriptions the client can fill at any pharmacy or lab the same day.
Form sealed. Once all required results and vaccinations are documented, Dr. Jetty signs and seals Form I-693. The sealed envelope is given to the client for inclusion with their USCIS filing.
You stay informed. With client authorization, we can send a copy of the completed I-693 directly to your office.

What We Need From You

Client’s full legal name, phone, and email
Any special considerations (pregnancy, prior I-693 issues, documented medical conditions)
If applicable, a brief note on filing deadlines so we can prioritize scheduling

Fee & Billing

$550 per applicant, payable by the client at the time of the exam. We accept credit/debit, bank transfer, and cash. The fee includes the physician-led exam, intake coordination, review of records, prescriptions, and same-day sealing of Form I-693.

Outside laboratory tests, vaccinations, and chest X-ray imaging are billed separately by the external provider directly to the client. We do not mark up or process outside services.
