What Sets Our Immigration Physical Apart
- Physician-performed, start to finish. Dr. Jetty personally conducts your exam, interprets your results, and signs your I-693 — no mid-level handoff, no assembly-line approach.
- Intake forms sent ahead. You arrive prepared; we transcribe directly into your I-693 rather than redoing paperwork at the visit.
- Prescriptions written same day. If labs (QuantiFERON-TB, syphilis, gonorrhea) or vaccines are needed, you leave with ready-to-use prescriptions for LabCorp, Quest, or any pharmacy. No return trip.
- Advance chest X-ray prescription provided. If your TB blood test is positive, you can go directly to imaging without a second visit.
- Same-day sealing whenever possible. Dr. Jetty signs and seals your form the moment all required documentation is in hand.
What is the I-693 Immigration Medical Exam?
Form I-693, Report of Medical Examination and Vaccination Record, is required by U.S. Citizenship and Immigration Services (USCIS) for applicants seeking adjustment of status to lawful permanent resident. The exam must be performed by a civil surgeon designated by USCIS. Dr. Jetty serves applicants from Princeton Junction, Princeton, Plainsboro, East Windsor, West Windsor, Hamilton, and across central New Jersey.
Who Needs an I-693 Examination?
- Applicants filing Form I-485 (Adjustment of Status) for a green card
- Applicants whose consular physical is more than two years old
- Applicants whose prior I-693 has expired or was incomplete
- Refugees, asylees, and VAWA applicants where USCIS has requested it
What the Examination Includes
Per the current USCIS Technical Instructions for Civil Surgeons:
- Medical history review — past medical, surgical, mental health, substance use
- Physical examination — vital signs, head-to-toe assessment
- Communicable disease screening — tuberculosis (QuantiFERON-TB Gold Plus), syphilis, gonorrhea when age-appropriate
- Vaccination assessment — review of your records against the USCIS vaccination requirements, with prescriptions for any age-appropriate vaccines you still need
- Mental health and substance use screening — as required by USCIS
- Completion of Form I-693 — signed and sealed the same day whenever possible
What to Bring to Your Appointment

- Any prior chest X-ray reports or TB test results
- List of current medications and known allergies
- Eyeglasses or hearing aids if you use them

Fees
$550 per applicant for the complete immigration medical examination.
This fee covers the entire physician-led exam, intake coordination, in-office review, prescriptions for any required labs and vaccines, the chest X-ray prescription (used only if needed), and same-day signing and sealing of Form I-693 by Dr. Jetty.
Outside laboratory tests (TB, syphilis, gonorrhea, immunity titers), vaccinations, and chest X-ray imaging — if required by USCIS or indicated by your results — are billed separately by the lab, pharmacy, or imaging center and are not included in the $550 exam fee. We provide the prescriptions; you handle those directly with the external provider.
Frequently Asked Questions
How long is the I-693 valid?
Per USCIS policy effective 2024, the I-693 is valid indefinitely from the date the civil surgeon signs it, provided it was properly completed and sealed within the required timeframe prior to filing or an adjudication decision.
Do I need to be fasting?
No. The exam does not require fasting. You can eat and drink normally before your appointment.
What if I do not have my vaccination records?
Please do your best to find them. If some records are missing, Dr. Jetty will determine which vaccines are age-appropriate and either verify your immunity with a blood test or write a prescription for the missing vaccine(s). You can complete missing vaccinations at your regular pharmacy, bring back the record, and Dr. Jetty will update the I-693.
What if my TB test is positive?
If your QuantiFERON-TB Gold Plus blood test returns positive, you will need a chest X-ray to rule out active tuberculosis. We provide a chest X-ray prescription in advance so you can go directly to an imaging center without a second visit — the prescription is only used if the blood test is positive.
Can my family schedule together?
Yes. Please let us know when scheduling so we can block adequate time for everyone.
Do you perform physicals for children and infants?
Yes. Dr. Jetty performs I-693 examinations for applicants of all ages, including infants, children, teenagers, and adults.
